Prescriptions

To order repeat prescriptions, please complete and submit the form on this page. You may also renew prescriptions by filling out a form available at reception, in the local chemists, or for download here. The form can be posted to the surgery or dropped off.

Your prescription will be ready in two working days, and can be collected from the chemist of your choice. Please remember to indicate your choice on the form.

 

If you wish to have new medications prescribed, please make an appointment to see a doctor.
